About the Role:

We are seeking dedicated Nurse Practitioners and Physician Assistants to join our integrated care team. As a key member of this team, you will work alongside a Registered Nurse and a Social Worker to provide personalized, compassionate medical care to patients with chronic kidney disease (CKD) and/or end-stage renal disease (ESRD/ESKD). This is a unique opportunity to make a real difference in the lives of patients who often face multiple serious health challenges. This is a hybrid position with in-home and virtual components.

What You'll Do:

Conduct comprehensive assessments, including annual wellness exams, both in patients' homes and virtually.
Counsel and educate patients and their families about available benefits and programs to support healthier living.
Document patient information, including chief complaints, diagnoses, medical history, reviews of systems, examinations, medications, allergies, assessments, and plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation promptly and accurately.
Maintain high-quality care standards.
Deliver evidence-based, timely care that reduces avoidable hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
Prescribe medications, order tests, and collaborate with patients' physicians.
Work to improve patient quality outcomes, which will be measured and reported.
Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Make assessments of patients' health status.
Develop and implement treatment plans.
Follow up and evaluate patients' status.
Perform other duties as assigned.
What You'll Bring:

Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs and information systems.
Excellent communication skills, both verbal and written.
Understanding of federal and state regulations, including HIPAA and OSHA.
2+ years of direct patient care experience.
Experience in managed care/IPA/health plans is preferred.
Experience conducting annual wellness visits or similar comprehensive visits (virtually or in-home).
Bilingual (English/Spanish) skills are highly desirable.
