Key Responsibilities
Conduct comprehensive patient assessments, including annual wellness exams, both in-home and via telehealth.
Educate patients and their families about available health programs and preventive care.
Maintain detailed and up-to-date medical records, including diagnosis, treatment plans, and medication management.
Coordinate care with primary care providers, specialists, and ancillary services.
Ensure timely documentation and adherence to quality-of-care standards.
Deliver evidence-based care to improve health outcomes and reduce hospitalizations.
Prescribe medications, order diagnostic tests, and collaborate with physicians to optimize patient care.
Participate in integrated care team meetings and facilitate patient education initiatives.
Close gaps in care by scheduling preventive screenings and tests.
Maintain compliance with all federal and state healthcare regulations, including HIPAA and OSHA.
Stay current with best practices in disease prevention, diagnosis, and treatment planning.
What We're Looking For
Bilingual (English/Spanish) candidates highly preferred
Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license
Board Certification - (NP: ANCC/AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Ability to work autonomously with minimal supervision
Proficiency in medical technology - Electronic medical records, data management systems, and virtual care platforms
Excellent communication skills - Comfortable interacting with patients, families, and healthcare teams
Knowledge of healthcare regulations - Understanding of HIPAA, OSHA, and managed care protocols
Minimum 2 years of direct patient care experience
Experience in Managed Care/IPA/Health Plan settings preferred
Prior experience conducting annual wellness visits or in-home assessments is a plus
