Your Role & Responsibilities
Perform comprehensive annual wellness exams both in patients' homes and via telehealth
Educate patients and families on available healthcare programs and lifestyle improvements
Document detailed patient histories, assessments, diagnoses, and treatment plans
Coordinate care with primary care physicians, specialists, and ancillary services
Maintain high standards of care, ensuring compliance with best practices and regulations
Prescribe medications, order diagnostic tests, and collaborate with physicians
Proactively close gaps in patient care by facilitating preventive screenings and treatments
Engage patients in educational resources to support their long-term health management
Participate in integrated care team meetings and contribute to care planning
Monitor and enhance patient health outcomes, prioritizing satisfaction and quality of life
Attend occasional training sessions or meetings outside of your primary work location
What We're Looking For
Bilingual (English/Spanish) candidates strongly preferred
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
Board certification required (NP: ANCC/AANP; PA: NCCPA)
Unrestricted DEA certificate
Ability to work independently with minimal supervision
Proficiency in medical instruments, electronic health records (EHR), and telehealth platforms
Strong verbal and written communication skills for interacting with patients, providers, and healthcare partners
Knowledge of HIPAA, OSHA, and federal/state healthcare regulations
Minimum two years of direct patient care experience
Experience in managed care, IPA, or health plan settings is highly desirable
Prior experience conducting annual wellness visits or similar in-home/virtual assessments preferred
