We're seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to deliver personalized medical care, primarily to individuals with Chronic Kidney Disease (CKD) and/or End-Stage Renal Disease (ESRD/ESKD). Your Impact:
You'll play a crucial role in improving the lives of individuals with chronic conditions by providing comprehensive, in-home and virtual care. Your expertise will be vital in:
Conducting thorough assessments, including annual wellness exams, in patients' homes and via telehealth.
Educating and counseling patients and their families about available benefits and programs to promote healthier living.
Maintaining accurate and detailed patient records, including chief complaints, diagnoses, medical history, and treatment plans.
Coordinating care with primary care physicians, specialists, and ancillary services.
Delivering evidence-based, timely care to reduce hospitalizations and enhance patient satisfaction.
Prescribing medications, ordering tests, and collaborating with supervising physicians.
Facilitating preventative monitoring and diagnostic testing.
Providing education and support to patients regarding health resources.
Participating in integrated care team meetings.
Providing disease diagnosis, and prevention education.
Creating and implementing treatment plans.
Following up and evaluating patient's status.

Roles and Responsibilities:
Perform comprehensive assessments, including annual wellness exams, in-home and virtually.
Document all patient encounters accurately and efficiently.
Coordinate care with other healthcare providers.
Deliver patient education and counseling.
Prescribe medications and order diagnostic tests.
Monitor and evaluate patient progress.
Contribute to quality improvement initiatives.
Participate in team meetings.
Position Requirements:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management systems.
Excellent verbal and written communication skills.
Understanding of HIPAA and OSHA regulations.
Minimum of 2 years of direct patient care experience.
Managed Care/IPA/Health Plan experience is a plus.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
Bilingual (English/Spanish) highly preferred.
This position will be remote in state in which employed and will need to attend periodic training/meetings outside of that state.
